At your first appointment
The aim of your first appointment is to discuss your dietary concerns with your dietitian. Or if seeing our diabetes educators this will involve any concerns you have around diabetes self management.
This first appointment may include reviewing the information provided by your referring General Practitioner, Nurse Practitioner, or other health professional. Information may be given by a current Care Plan, current medication lists, current pathology results.
At your first appointment the Dietitian or Diabetes Educator may ask about your medical history including self blood glucose monitoring, you diet and complete a diet history, your social history and exercise history as standard information required to best advise and assist you in understanding your medical conditions. From this information a tailored holistic approach is taken. A very personalised diet and lifestyle plan will be provided and together achievable goals will be set.
Suggested follow up appointments will be made depending on the goals set and what you feel is achievable. This is the best way to ensure your original referral will achieve your goals as agreed with your GP in your GPs Care Plan and Team Care Arrangement (TCA) referral forms.
Appointments can be schedule to suit your work and family commitments and financial restraints.
