1. Do you take new patients?
Yes, we realise new families coming to Geraldton require a dietitian or diabetes educator. With three highly qualified dietitians and credentialled diabetes educators we will try to accommodate your requests.
2. How do I make an appointment?
Please call our friendly receptionist on 9964 5005 or 9964 4464 who will endeavor to accommodate your most suitable days and times to attend for an initial consultation.
3. Can I Self Refer?
Yes we are happy to accept self referrals and this indicates you are an independent thinker that is actively planning to review and make lifestyle changes to enjoy a better quality of life long term.
4. I play sport and would like a personalised eating plan and advice on competition sports nutrition?
Yes, Stacey is our Sports Dietitian Australia member dietitian. As an APD and SDA Stacey can tailor a plan for your sport including training, pregame planning and post game recovery foods that will give you the edge over your competitors or for personal best challenges. With up to date evidence based knowledge Stacey will also be able to do skin fold measurements, bio-metrical analysis and provide expert guidance for weight loss or weight gain together with medical nutritional therapy for any chronic diseases, eg, Coeliac disease, Chrons disease, etc.
5. Our team are interested in more nutritional education, can you help?
Yes, for sporting groups or for interest groups we can provide workshops or talks (lectures) on any topic around sports nutrition, general nutrition, nutrition for a specific sport, chronic disease eg Diabetes. Recently we worked with a gym to provide menu planning and shopping lists for focused eating.
6. What do I bring?
Bring your self and a support person if you feel you need a 'second pair of ears' to assist you to enable any lifestyle or medications changes you plan to make. If you have Private Health Insurance please bring with you your Private Health Insurance members card, eg HBF, HIF, Medibank, AHM, Teachers Union, etc.
If your General Practitioner, Doctor, Medical Specialist or Allied Health Professional has referred you please bring any referral paper work they have given you. Please bring any Care Plans that are current, medication lists or Team Care Arrangement paper work.
Your Medicare Card and your debit card will be required if you wish to claim Medicare Benefit Schedule for the Team Care Arrangement.
Any identifying cards such as your Dept Of Veteran Affairs card, Aged Pension Card and Health Care Card will assist us to quickly process your payment and claim on the day of treatment, and you may be eligible for a bulk billing rate with these concession cards.
7. Can I pay on the day?
On completion of your dietetic or diabetes education appointment, you will receive an account to settle on the day. To make this easier we have HICAPS facilities for private health funds to issue your rebate and you pay the gap.
Our service offers Medicare Benefit Scheme when the appropriate paper work is presented prior to your appointment. To use MBS you will require a current Care Plan from your General Practitioner and a Team Care Arrangement (TCA) which offers up to 5 visits each calendar year.
For Department of Veterans Affairs (DVA) patients, the paper work will be completed as treatment progresses to keep DVA fully aware of your care.
We have EFTPOS for easy payments on the day.
